Frederick B. Axelrod, M.D., MBA
President/Chief Executive Officer and Medical Director
A board-certified clinical pathologist, Dr. Axelrod received his medical degree and performed his Anatomic and Clinical Pathology training at Emory University School of Medicine and Emory University Affiliated Hospitals in Atlanta, GA. He also completed a fellowship in transfusion medicine at Cedars-Sinai Medical Center in Los Angeles, CA and acquired his MBA at the J.L. Kellogg Graduate School of Management, Northwestern University, Evanston, IL. He has served as the chief medical officer for Indiana Blood Center and Senior Director/Division Head, Medical and Clinical Affairs with Abbott Laboratories Diagnostic Division in Abbott Park, IL and has worked in transfusion medicine since 1987. He serves on the boards of directors for Desert Regional Medical Center and Coachella Valley Economic Partnership.
Joe Chaffin, M.D.
Vice President, Chief Medical Officer
Dr. Chaffin is a graduate of the Loma Linda University School of Medicine in Loma Linda, CA. He served his country for nine years as an Army physician, primarily practicing Anatomic and Clinical Pathology at Walter Reed Army Medical Center in Washington, DC. Dr. Chaffin has an extensive background in hospital pathology and Transfusion Medicine, including practice at multiple hospitals in Northern Colorado and, most recently, at Cedars-Sinai Medical Center in Los Angeles, CA. He is a board-certified Blood Bank/Transfusion Medicine specialist and is widely known for his educational efforts through his award-winning personal website, “Blood Bank Guy” (www.bbguy.org). Dr. Chaffin is the primary Transfusion Medicine educator for a national pathology board review course.
Scott Miller, CPA, MBA
Vice President, Business Development / Chief Financial Officer
Scott has more than 25 years’ experience in the healthcare field and has served as chief financial officer for organizations recording up to $1 billion in annual revenues. He joined LifeStream in October 2010 and has overseen all finance and purchasing functions, and plays a key role in contractual matters. He was named to his new position in April 2017 and has assumed responsibilities for Human Resources and Community and Corporate Relations. Scott received his bachelor’s degree from Southern Missionary College in Tennessee and his MBA from Rollins College in Winter Park, FL.
Kevin Moore, MT (ASCP)SBB
Vice President, Operations
Kevin has more than 30 years’ experience in the blood banking industry and has an extensive background in managing manufacturing, distribution and immunohematology reference laboratory service functions with local and national blood centers including the Blood Bank of Hawaii and American Red Cross. Kevin received his bachelor’s degree in Medical Technology from Purdue University and his specialist in blood banking certification from Michael Reese Hospital and the Blood Center of Northern Illinois. He was named to his present position in April 2017 and oversees Donor Operations, Recruitment, Manufacturing, Facilities, Reference Laboratory, and Marketing Communications.
Julianna Quarles, RN
Vice President, Technical and Clinical Affairs
Julianna began her career with Blood Bank of San Bernardino and Riverside Counties in 1995 as a mobile staff nurse, shortly after graduating from nursing school. She subsequently assumed managerial responsibilities, including staff nursing supervisor and apheresis supervisor; and Manager of Donor Center Operations and Telerecruitment. Julianna was named Vice President/Technical Operations in 2008 and named to her present post in April 2017 and oversees Medical Surveillance and Employee Development, Information Technology, the National Marrow Donor Program, and licensed nursing and advanced collections training.
Bob Bayer, MT(ASCP)BB MBA
Sr. Quality Director, Quality and Regulatory Affairs
Bob joined LifeStream in February 2012 as Project Director for Quality Assurance. He has an extensive leadership background in the blood banking industry. Senior executive posts include National Director of Quality Assurance and Regulatory Affairs for HemaCare Corporation; CEO for the American Red Cross National Testing Laboratory in San Diego, CA; and Regional Quality Assurance Manager for Blood Systems, Inc., located at Blood Centers of the Pacific in San Francisco, CA. Bob earned his MBA at Loyola College, Baltimore, MD and his bachelor’s degree in medical technology from State University of New York at Buffalo.
